Wellness Rx

We have partnered with local farms and retailers to make fresh, nutritious foods more available to patients with chronic disease. The Wellness Rx nutrition program aims to increase access to nutrition education and affordable produce for patients with or at risk of diabetes.

Patients with food-related illnesses like diabetes are encouraged to take. Ask your primary care provider about a referral and take the first step by meeting with our certified diabetes educator. You will be “prescribed” vouchers for fresh fruits and vegetables and begin your journey to better health.

All farmers markets in Essex County and many in Clinton County are now accepting program vouchers.

The Village Meat Market in Willsboro and Pray’s Family Farms in Plattsburgh also accept Wellness Rx vouchers.
