The Elizabethtown Community Health Center is part of The University of Vermont Health Network - Elizabethtown Community Hospital’s network of community health centers in Essex County. The health center is run by a dedicated care team that brings a wealth of experience and connection to all that ECH and the University of Vermont Health Network has to offer. If you need primary care, follow-up care or a consultation before referral to a specialist, we can help.

Preventive Services, Diagnostic Tests & Treatment

Elizabethtown Community Health Center provides preventive services, diagnosis, and treatment to patients of all ages. Our care teams take care of a wide range of conditions across the lifespan. In the event that our clinic staff cannot treat a certain condition at a community health center, they will provide referrals to services within the ECH and UVM Health Network.

Prescriptions

If you are followed routinely, we can usually refill your medication without an extra office visit.

You can fill your prescription by calling the office at (518) 873-6896 and choosing the option for prescriptions. You can also request refills through the Patient Portal.

We require 48-72 hours of notice for regular prescription requests. Please give the exact name or spelling of the medication, the strength, how and when taken, and the amount required.

Appointment scheduling and cancellation 

At the Elizabethtown Community Health Center, we try our best to get our patients in to see a provider as soon as possible. Appointment availability often depends on the type of appointment and the provider’s schedule.

We work to provide openings every day for urgent needs. Please call the evening before and leave a message or call early in the morning for availability.

The health center requires two hours prior notice to cancel.

Availability and reporting of results

Although many laboratory and radiology test results are reported quickly, the results must still be forwarded to your provider for review. Staff will notify you of your results as soon as your provider has given them instructions to do so. If you do not hear from us in one week, please call the office.

If you wish us to leave a message with a family member or friend regarding test results, appointments, or other aspects of your care that we might contact you about, you may sign a release to be kept in your chart designating the individual(s).

How can I obtain copies of my records?

Records may be requested via an approved HIPAA release form. All record requests will be filled within 10-14 working days after receipt of payment. There may be a charge for records.

If you are transferring care to another physician, we will send the last years' records free of charge to your new physician, but only for the first request. If you transfer to another physician after that, there will be a charge for all records.

The Health Center participates with numerous plans and insurances. If you belong to an HMO, (Health Maintenance Organization), you need to designate the Health Center physicians as your primary care provider. Co-payments are due at the time of your visit.
