Patient & Family Support Program 

The Patient & Family Support Program at Elizabethtown Community Hospital ensures that patients and their families have access to the equipment and transportation they need to maintain their health and wellness. Our goal is to reduce barriers to health improvement.  

We can provide the following items: 

• Blood pressure monitor  
• Pulse oximeter  
• Scale  
• Pill boxes  
• Lake Champlain Ferry Passes  
• Stewarts gas cards 

All Elizabethtown Community Hospital patients are eligible for support. Both Emergency Departments, the inpatient unit and all six health centers carry supplies for our patients. Speak with your provider about accessing the Patient & Family Support Program.
