Swing Bed Program

Our Swing Bed Rehabilitation Program is designed to provide patients with individualized, in-hospital care and physical rehabilitation to help reach their optimal level of functioning. This sub-acute care is designed for patients who are discharging from an acute level of care, but need temporary, additional care prior to returning to their previous living environment. 

Therapy Offered

Occupational Therapy: This type of therapy focuses on "what matters" to an individual. This may include activities such as cooking, dressing, maintaining personal care, and returning to meaningful hobbies. Occupational therapy may also include a home visit to evaluate an individual's living environment, adapting environment's, recommending adaptive equipment and training in its use. 

Recreation Therapy: Recreation therapy provides adapted recreational opportunities that encourage independent living skills and improve quality of life. Activities are offered daily to swing bed patients. We offer a large variety of independent, one-on-one, and group activities. Activities are modified according to individual care plans. Therapeutic activities include: sports, crafts, dance, visits by pet therapy animals, stress reduction techniques, music, games, baking, community outings, and much more!

Physical Therapy: This therapy involves working with individuals to expand and restore motion. Interventions may include focus on strength, range of motion, balance, walking and climbing stairs. We'll work collaboratively with each individual to tailor a treatment plan toward his or her personal goals and needs.

Frequently Asked Questions

How long will I be here?
Each patient is unique, and each length of stay varies. Our team of professionals will create a plan of care designed to suit your individual needs and goals. Our case manager will also work closely with you and your family to create an appropriate discharge plan for you. 

How often will I receive therapy?
One-on-one therapy is provided 5 to 6 days a week. Length of time for each session may vary from day to day. 

What should I bring from home?
We encourage you to bring comfortable clothing and proper footwear for your therapy sessions.

When will I see my physician?
Your physician will see you at least once weekly. Our experienced team of nurses and therapists can communicate with your physician at any time if concerns arise regarding your health status. It is also important to note that there is a Physician's Assistant on call in our Emergency Department 24 hours a day, 7 days a week.
